Claims Request Form

Please submit one form per invoice.

All claims must be submitted within 48 business hours of delivery. Claims submitted after the 48 hour time period are subject to Wild Eye Designs approval.

Please retain all damaged/incorrect merchandise and supporting documentation until your issue is resolved. A copy of this form will be emailed to you for your records.
